  1. Sponsor.  The Sponsor of this drawing by chance is Halifax Hospital Medical Center through its Nursing Shared Governance, whose principal place of business is 303 North Clyde Morris Boulevard, Daytona Beach, Florida 32114.

  1. Obtaining Entries.   Entries for the drawing may be obtained through the Official Drawing Website or, for employees of the Sponsor, through online payroll deduction.  Minimum suggested donation is $1 per ticket.  Alternatively, entrants may request to be entered in the drawing by appearing in person at the Sponsor’s principal place of business.  The Sponsor will not provide paper tickets for the drawing.  A valid email address is required for all entrants. Each entry may be obtained by one entrant only, in that entrant’s own name.  Sponsor reserves the right to limit the number of entries distributed to any one individual. Entries must be received by 10:00 am on May 15, 2026. No purchase or contribution is required to enter. Contact Nicholas Heiskell at 386.425.4224 for more information.

  1. Random Drawing.  A random drawing for each prize will be held at 303 North Clyde Morris Boulevard, Daytona Beach, Florida on May 15, 2026 at 12:00 pm from all eligible entries received. Winners need not be present at the time of the drawing. The Sponsor will notify all eligible winners within twenty four (24) hours of the drawing.  Notifications to winners will be delivered either via email or telephone using the contact information set forth on the drawing entry.  Winners must present a valid government-issued identification to claim prizes.

 

  1. General Conditions.  Sponsor assumes no responsibility for lost, late, incomplete or misdirected entries or email; or for any computer, telephone, cable, network, electronic or Internet hardware or software malfunctions or failures.  The entrant is responsible for monitoring all spam and junk email folders for notifications related to the drawing.  Winners must be of legal age to claim any prize containing alcohol and/or lottery tickets.  If the chosen winner of a basket containing alcohol and/or lottery tickets is not of legal age, the basket must be claimed by the winner’s parent or legal guardian.

  1. Prizes awarded.  The prize baskets to be awarded are available for viewing on the Official Drawing Website.  Prize baskets have been provided by Halifax Hospital Medical Center and its departments, employees, and/or medical staff members.
